Untreated Vein Disease: What Happens When You Keep Waiting?

Vein disease, also called chronic venous insufficiency (CVI), does not stay the same. Without treatment, it advances. And the further it goes, the more complicated it becomes to address.

Most people do not treat it because they do not realize it is getting worse. The symptoms feel manageable, life stays busy, and “I’ll get that checked” stays on the back burner. But the pressure building in those veins is quietly doing damage.

Here is what that progression looks like.

Stage by Stage: How CVI Advances

Stage 1: Discomfort without visible changes Heavy, tired, achy legs by end of day. Swelling in the ankles and calves that resolves overnight. Nothing visible on the surface yet, which is part of why people keep waiting.

Stage 2: Visible vein changes Spider veins and varicose veins appear. Varicose veins are not purely cosmetic — they signal that venous pressure has reached the point of physically distorting the vein walls.

Stage 3: Skin changes The skin around the lower leg and ankle begins to darken, thicken, or discolor. Chronic venous pressure causes red blood cells to leak into surrounding tissue. The area may feel itchy, tight, or tender.

Stage 4: Ulceration The skin breaks down and venous ulcers develop — open wounds near the ankle that are slow to heal and prone to infection. At this stage, treatment is significantly more intensive and results are less predictable.

What Treatment Actually Involves

No surgery. No hospital stay. Most procedures take under an hour in the office, with same-day return to normal activity. Treatment options include:

  • ClosureFast: radiofrequency energy used to close the damaged vein from the inside
  • VenaSeal: a medical adhesive that seals the vein without heat or incisions
  • Varithena: a prescription foam that causes the vein to collapse and be reabsorbed
  • Sclerotherapy: an injectable solution that collapses and fades the vein over time

Treatment is often covered by insurance when medically documented. The earlier the stage, the more straightforward the process.

Ready to Find Out Where You Stand?

A vein evaluation is non-invasive and takes about as long as a routine appointment. A duplex ultrasound maps blood flow in your legs, and by the end of the visit you will know whether vein disease is a factor and what your options are.

At Coastal Vascular & Vein Center, our team treats vein disease at every stage with minimally invasive, personalized care. Schedule your consultation and find out exactly where things stand.
